Who We Are and What This Policy Covers

Chasteen Counseling, PLLC (“us”, “we”, or “our”) operates the Chasteen Counseling, PLLC website at chasteencounseling.com (the “Site”, “Website”, or “Service”).

This page informs you of our policies regarding the collection, use and disclosure of Personal Information when you use our Site.  This policy does not cover any other aspects of the services provided by Chasteen Counseling, PLLC.

We will not use or share your information with anyone except as described in this Privacy Policy.

Information Collection And Use

We collect information (“information” or “data”) that you voluntarily submit to us through the Site, as well as data that is automatically provided to the Site by your web browser.

Collection of Personal Information

While using our Site, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you through a Secure Message Form that uses the Hushmail™ encrypted email service. Personally identifiable information (“Personal Information”) may include, but is not limited to:

  • Name
  • Email address
  • Telephone number
  • IP Address
  • Other data provided through the Secure Message Form

All Personal Information is collected through the encrypted Secure Message Form provided through the Hushmail™ encrypted email service (see the Hushmail™ terms of service and privacy policy here: www.hushmail.com). The Site does not collect Personal Information except through the Hushmail™ encrypted email service. No Personal Information is intentionally collected, routed, or stored by any mechanism or service provider except the Hushmail™ encrypted email service.

Collection of Other Information

We automatically collect non-personally-identifying information that your browser sends whenever you visit our Site (“Log Data”). This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our Site that you visit, the time and date of your visit, the time spent on those pages and other statistics.

Like most website operators, we automatically collect non-personally-identifying information of the sort that web browsers and servers typically make available whenever you visit our Site (“Log Data”). This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, language preference, referring site, the date and time of each visitor request, the pages of our Site that you visit, the time and date of your visit, the time spent on those pages, and other statistics. Our purpose in collecting non-personally identifying information is to better understand how our visitors use the Site.

Log Data includes information like Internet Protocol (IP) addresses. The Site collects the IP addresses of visitors to its website but does not allow users to login, does not automatically link the IP address to any particular consumer or household, and does not collect data that could reasonably link the IP address with a particular consumer or household. Therefore, we do not treat IP addresses as personal information or personally-identifying information.

If you perform a search within the Site, we may record information linking your IP address and other Log Data to the search performed. Some of the Personal Information you voluntarily provide to us may be considered Protected Health Information (PHI). We will not intentionally disclose your Personal Information to third parties except as otherwise described in this policy and our HIPAA Notice of Privacy Practices for PHI.

We may disclose aggregate non-identifying information only where no individual is identified for several purposes, including solving technical problems with the Site, improvement of the services we provide, or to customize the content and layout of the Site.

Cookies are files with small amount of data, which may include an anonymous unique identifier. Cookies are sent to your browser from a web site and stored on your computer’s hard drive.

Our Site is not directed at children under the age of 13 (“Children” or “Child”). We do not knowingly collect personally identifiable information from Children. If you are a parent or guardian and you are aware that your child has provided us with Personal Information, please contact us. If you are a Child, you may use the Site only with the involvement of a parent or guardian. If we are notified that a Child has provided us with Personal Information without the involvement of a parent or guardian, we will delete such information from our servers.

We follow additional policies and procedures beyond what is described here when handling Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state laws. All Chasteen Counseling, PLLC clients receive a copy of this notice through the client portal when initiating services.

We may update our Privacy Policy from time to time. We will notify you of any changes by posting the new Privacy Policy on this page. You are advised to review this Privacy Policy periodically for any changes. Changes to this Privacy Policy are effective when they are posted on this page.
